Registered Nurse Case Manager

$2080.80/week - Job Summary: Provide clinical oversight and coordinate patient care as a Registered Nurse specializing in Case Management within a hospital setting. Collaborate with healthcare teams to ensure effective discharge planning and resource utilization to promote positive patient outcomes. Responsibilities: Assess patient needs and develop individualized care plans in collaboration with multidisciplinary teams. Coordinate discharge planning and facilitate communication between patients, families, and healthcare providers. Monitor patient progress and adjust care plans accordingly to meet clinical goals. Document patient interactions, assessments, and care plans accurately in electronic health records. Educate patients and families about post-discharge care and community resources. Comply with hospital policies, safety standards, and regulatory requirements. Qualifications: Current RN license in the state of North Carolina or eligible for licensure. Minimum of 2 years of experience in hospital case management or relevant clinical setting. Strong assessment, communication, and documentation skills. Ability to multitask and prioritize in a fast-paced environment. Proven ability to build trust with patients, families, and healthcare team members. Bilingual skills preferred but not required.
