Home Health Occupational Therapist (OT) - West Portland, OR

This is a contract position with an anticipated length of 13 weeks.

Full-time, Day Shift: 5x8-hour shifts (08:00 AM - 04:30 PM).

West Portland, Oregon, and surrounding areas.

We are seeking a licensed Occupational Therapist (OT) to join our Home Health team in West Portland, OR. The OT will provide valuable therapy services to patients in their homes, focusing on rehabilitation and skill development for improved daily living.

Responsibilities:

• Conduct patient evaluations and develop individualized treatment plans.
• Implement therapeutic interventions to promote functional independence.
• Educate patients and families on home health care techniques and tools.
• Document patient progress and modify treatment plans as needed, utilizing OASIS standards.
• Assist patients with adaptive equipment and home modifications.

Required Experience / Certifications / Licensure:

• Current Occupational Therapist license in Oregon.
• Minimum of 1 year of skilled home health experience is required.
• BCLS certification is required.
• Experience with OASIS documentation is preferred.
• Reliable transportation for travel between patient locations.
