ALL RTO MUST BE FINALIZED AT TIME OF SUBMISSION No new RTO after submission. NO ADDITIONAL RTO OR SCHEDULE REQUESTS CAN BE REQUESTED POST SUBMISSION, POST OFFER OR POST START/ONSITE No RTO allowed the first week of an assignment. License Information: MO: Submit without license OK; requires MO or compact (no issuance if tied to another compact state). IL: License in hand required at time of submittal.
