Duration: 6 Months Contract

Location: Poughkeepsie, NY 12601

Shift Timings: Monday –Friday (7:30 am - 4:00 pm)

Pay: USD40/hr-USD43/hr

Position summary: Provide individual and family therapy, crisis intervention, and case management for a caseload of 25–30 K–12 students, while collaborating with school staff, telehealth providers, and outside agencies. Participate in treatment planning, interdisciplinary meetings, and discharge coordination to support youth and family mental health needs.

• Maintain a caseload of 6 children in the IDT program within the elementary classroom, kindergarten - 6th grade.
• Participate in IDT admissions with child, family, MD, and school liaison.
• Provide Weekly Individual Therapy.
• Provide Daily Group Therapy.
• Provide Family Therapy and Crisis Intervention as needed.
• Facilitate Transition Meetings for each student on their caseload that include the student and family, IDT teacher, school liaison and other professionals from the student's home district.
• Collateral involvement with participating schools and agencies.
• Function as part of an interdisciplinary treatment team.
• Participate in weekly Treatment Team meetings.
• Advocate for youth and families to obtain community-based linkages with additional supportive services that meet their needs.
• Maintain timely medical record documentation.
• Facilitate thorough discharge planning and complete discharge follow-u.
• Collaborate with school liaisons throughout Dutchess County to facilitate the referral and screening process.
