Job Description
TITLE
Home Health RN - Home Health

SCHEDULE
- Shift: 8-hour days only
- Weekends: Not required
- On-call: No

ASSIGNMENT DETAILS
- Patient Load / Ratios: 1:1; may visit up to 6 patients/day
- Guaranteed Hours: Facility may cancel up to one shift per two-week pay period

REQUIREMENTS
- Minimum of 1 year of skilled home health RN experience
- OASIS experience required
- Licensure: OR & WA RN licensure required (pending licenses accepted)
- Required certifications: BCLS
- Key clinical skills: start/maintain IVs, phlebotomy, medication administration (IM/IV/subQ), admission/start-of-care, recertification, resumption of care, discharge, pain assessment/management, infection control

ROLES & RESPONSIBILITIES
- Provide direct patient care to adults and geriatric patients in a home health setting.
- Administer medications and treatments as prescribed.
- Perform assessments and develop care plans based on patient needs.
- Educate patients and families on disease management and care techniques.
- Ensure compliance with federal and state regulations regarding home health care.
- Document patient care and progress accurately in the required systems.

COMPLIANCE / NOTES
- Orientation: 40 hours
- Modules (~16 hours) required pre-start and annually
- Parking: Free
- Scrubs: Business casual preferred; scrubs allowed (no jeans); no specific color required
