Hospice RN Case Manager — Weekdays, No On-Call

JourneyCare Hospice in Northbrook, IL is seeking a compassionate Hospice Registered Nurse (RN) Case Manager for a full-time role. You will manage a dedicated hospice caseload, perform patient assessments and care planning, and coordinate with physicians and interdisciplinary team to deliver comfort-focused end-of-life care. This role follows a Monday–Friday schedule (8:30 AM–5:00 PM) with no on-call, one weekend rotation every 12 weeks and one summer/winter holiday. #J-18808-Ljbffr Addus HomeCare
