About AbrazarAbrazar (Spanish for “to embrace”) is a nonprofit direct-service organization that has addressed inequities in the social drivers of health since 1975. Position SummaryAbrazar is seeking a bilingual, community-centered Community Enrollment and CHW Manager to lead our CalOptima Community Enrollers work and help develop Community Health Worker (CHW) services across Abrazar. This non-clinical position combines direct Medi-Cal enrollment, community outreach, program management, partner coordination, staff training, resource navigation, and CHW service development.
The immediate funding focus is the First 5 Orange County and CalOptima Health Community Enrollers Program through June 2028. The Manager will prioritize Abrazar’s assigned Engaged Neighborhoods—Growing Up Garden Grove and the Santa Ana Early Learning Initiative—while also meeting families where Abrazar is located and works and pursuing eligible outreach opportunities through hospitals, home visiting, developmental playgroups, childcare, Kid Builders, partner networks, and other community venues.
The long-term opportunity is to help Abrazar turn underused CHW services into a coordinated, compliant, and sustainable program. The Manager will work with the CalAIM team and other departments to identify appropriate members, strengthen referrals, deliver and document CHW services, train staff, improve billing readiness, and demonstrate the value of CHW-based navigation and light-touch case management beyond enrollment alone.
This position has shared reporting responsibilities. Chelsea Leyva, HVP Director, provides leadership for the First 5 OC Community Enrollers contract, Engaged Neighborhood partnerships, outreach milestones, and family-service connections. Lisa Nguyen, CalAIM Director, provides leadership for CHW service development, CalAIM compliance, documentation, billing readiness, and integration with Abrazar’s CalAIM team. The Manager is expected to maintain clear communication with both directors and coordinate priorities across the two areas.
Compensation and Benefits• $25.00–$27.00/hour: Candidates meeting minimum qualifications with relevant community outreach, benefits enrollment, healthcare navigation, CHW, or direct-service experience.
• $27.01–$30.00/hour: Candidates with demonstrated Medi-Cal enrollment, program coordination, community partnership, staff training, CHW service delivery, or team-lead experience.
• $30.01–$32.00/hour: Candidates with advanced directly relevant experience in program management, staff supervision, CHW operations, documentation and billing readiness, contract performance, or other highly relevant preferred qualifications.
• Vacation: 12 days per year, accrued monthly after three months.
• Health leave: Up to 56 hours per fiscal year after three months.
• Holiday pay: 10 days per year after three months.

Essential ResponsibilitiesCommunity Enrollment Program Leadership• Lead daily implementation of Abrazar’s Community Enrollers Program and remain actively involved in enrollment, renewal, outreach, education, referrals, and follow-up.
• Maintain a monthly work plan, community-event calendar, partner schedule, enrollment appointments, outreach strategy, and progress dashboard.
• Coordinate regularly with First 5 OC, CalOptima Health, CHIOC, Engaged Neighborhood partners, and other participating agencies.
• Prioritize Growing Up Garden Grove and the Santa Ana Early Learning Initiative while developing outreach opportunities in Abrazar’s service area and other eligible community settings.
• Keep the shared tracker current; attend required check-ins; complete accurate monthly High 5 reporting; and promptly address performance gaps.
• Coordinate complex applications and enrollment surges with CHIOC and other available technical-assistance partners.
Contract Outcomes and Accountability• Participate in at least five community events each month.
• Complete at least 25 Medi-Cal applications or renewals each month.
• Provide face-to-face outreach and education to at least 100 unduplicated people each month.
• Connect at least nine families each month to continued services such as Healthy Steps, home visiting, fatherhood programs, developmental playgroups, and other appropriate resources.
• Identify and support two First 5 OC-funded partner-agency staff each month to become certified enrollers.
• Track locations, Engaged Neighborhood participation, outreach contacts, applications, referrals, trained enrollers, outcomes, and follow-up needs.
• Participate in evaluation, continuous quality improvement, annual planning, data collection, and all required program meetings.
Medi-Cal Enrollment and Benefits Navigation• Assist community members with Medi-Cal applications, renewals, document collection, status follow-up, and coverage-maintenance education.
• Explain application requirements and changing eligibility or renewal expectations in clear, culturally, and linguistically appropriate language.
• Provide accurate referrals and warm handoffs for CalFresh, CalWORKs, Covered California, VITA/ITIN, healthcare, developmental, family-support, and other community resources.
• Maintain appropriate certifications and complete required enrollment, privacy, security, and program training.
• Protect confidential information and maintain accurate records of appointments, applications, contacts, referrals, and outcomes.
CHW Program Development and Service Delivery• Maintain a limited CHW caseload and provide person-centered assessment, health education, system navigation, advocacy, care planning, follow-up, and connection to health and social services.
• Work with CalAIM and other Abrazar departments to identify members and activities appropriate for CHW services and establish effective internal referral pathways.
• Develop practical CHW workflows, screening and referral tools, documentation templates, service standards, productivity expectations, and follow-up processes.
• Train and coach staff on CHW eligibility, allowable services, documentation, referrals, member engagement, and billing readiness.
• Review CHW documentation for completeness and collaborate with program, finance, billing, and compliance staff to address gaps and support accurate claims.
• Monitor CHW referrals, services, outcomes, productivity, and revenue and recommend improvements that increase appropriate utilization and sustainability.
• Serve as a cultural liaison and trusted navigator for members facing healthcare, behavioral-health, housing, transportation, language, benefits, or other access barriers.
Community Partnerships and Outreach• Build trusted relationships with families, schools, early-childhood networks, healthcare providers, hospitals, community organizations, faith communities, public agencies, and neighborhood leaders.
• Plan and lead outreach events, presentations, enrollment appointments, group education, and follow-up activities, including occasional Saturdays and evenings.
• Develop culturally appropriate outreach materials and coordinate promotion through Abrazar programs and partner networks.
• Represent Abrazar professionally and ensure community members receive respectful, responsive, and accurate assistance.
Minimum Qualifications• Bilingual proficiency in English and Spanish or English and Vietnamese.
• Bachelor’s degree in Public Health, Social Work, Human Services, Public Administration, Community Health, or a related field. A directly relevant combination of education and experience may be considered.
• At least three years of relevant experience in community outreach, benefits enrollment, healthcare navigation, CHW services, case management, care coordination, or social services.
• At least one year of staff leadership, program coordination, team-lead, training, or comparable responsibility.
• Experience assisting people with Medi-Cal enrollment or renewals, public-benefit applications, healthcare navigation, or comparable eligibility processes.
• Demonstrated ability to plan community events, build partnerships, meet numeric deliverables, and maintain accurate trackers and reports.
• Strong documentation, follow-through, public speaking, cultural humility, and interpersonal communication skills.
• Ability to safeguard protected and confidential information in accordance with HIPAA and agency requirements.
• Proficiency with Microsoft Office, particularly Excel, and comfort learning enrollment, case-management, billing, and reporting systems.
• Ability to work occasional evenings and Saturdays and travel throughout Orange County.
Preferred Qualifications• Current or prior certification as a Medi-Cal enroller, Covered California Certified Application Counselor, CHW, promotor/a, or comparable navigator.
• Experience with CalOptima Health, First 5 OC, BenefitsCal, Medi-Cal renewals, CalFresh, CalWORKs, Covered California, VITA/ITIN, or Orange County benefit systems.
• Experience developing CHW programs, workflows, documentation, billing practices, staff training, or cross-department referral systems.
• Knowledge of CalAIM CHW services, Medi-Cal documentation and billing requirements, and the distinction between CHW, ECM, and other care-management services.
• Experience working in Garden Grove, Santa Ana, Westminster, Midway City, or with Latino, Vietnamese, immigrant, low-income, and families with young children.
• Training in Motivational Interviewing, Trauma-Informed Care, Harm Reduction, Crisis Intervention, De-escalation, health education, or related practices.
• Experience with ETO or comparable case-management software and intermediate or advanced Excel skills.

Mandatory Employment and Driving Requirements• Verification of authorization to work in the United States.
• Current CPR/First Aid certification or willingness and ability to obtain it.
• Successful completion of tuberculosis, background, and drug screenings as conditions of hire.
• Reliable transportation, a valid driver’s license, appropriate automobile insurance for any personal vehicle used for work, and the ability to qualify for Abrazar’s insurance coverage.
