Key Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Provide direct patient care in an outpatient setting, typically seeing 2 patients per hour with sessions of 45 minutes or more; initial evaluations and safety patients receive a full hour of one-on-one care
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities
• Serve as an educator to patients, families, students, and coworkers; participate in student programs as assigned, providing appropriate supervision and completing student evaluations
• Adhere to discipline-specific state practice acts, regulatory requirements, and professional codes of conduct; maintain professional licensure in good standing

Required Qualifications

• Active Physical Therapist (PT) license in good standing in the state of Colorado
• Ability to complete comprehensive patient evaluations and develop clinically reasoned treatment plans
• Proficiency in medical record documentation compliant with state, federal, and intermediary guidelines
• Commitment to patient and employee safety standards and safe patient handling practices

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring PT students
• Familiarity with performance improvement methodologies and quality initiatives
