Job Description
Registered Nurse - Case Manager

Schedule

Day shift, 5x8 hours (08:00 - 16:30). Required to work every other weekend.

Assignment Details

Start date: ASAP. The position requires 2 years of acute care experience. Call is not required. The traveler may need to float between like units based on need. Local candidates are accepted. Please include all Requests Time Off (RTO) at the time of submittal. Charting will be done using Epic. Free parking is available on the first day, with assignment parking provided during orientation.

Requirements

• Active RN license
• 2 years of acute care experience required
• EMR system proficiency: Epic

Responsibilities

• Assess patient needs and develop a comprehensive case management plan.
• Coordinate patient care across multiple disciplines and settings.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families about available resources and treatment options.
• Collaborate with healthcare team members to ensure quality patient care.
