Registered Nurse (Community Living Center)

The Registered Nurse (Community Living Center) is within the Nursing (Geriatrics and Extended Care) service at the Boston VA Healthcare System located in Brockton, MA. It is permanent and full-time at 40 hours per week. The Community Living Center (CLC) Registered Nurse (RN) is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care. Duties include, but are not limited to:

• Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care;
• Administers medications and procedures per established policies and guidelines;
• Influences care outcomes by collaborating with members of the interdisciplinary team;
• Participates in the unit/program level quality improvement process and initiatives as well as customer service programs;
• Determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care;
• Formulates a plan of care based on nursing assessments;
• Engages patients, families, and other caregivers to incorporate knowledge, values, and beliefs into care planning without judgement or discrimination;
• Delivers consistent care and improve patient outcomes;
• Demonstrates positive, effective communication skills and professional behaviors that promote cooperation and teamwork with internal and external customers; and
• Performs other duties as assigned.
