Position Title: Physical Therapy Assistant (PTA)

Location: Sligo Creek Healthcare

Summary

Sligo Creek Healthcare is seeking a motivated and compassionate Physical Therapy Assistant (PTA) to join our skilled rehabilitation team. The PTA will work under the direction of a licensed Physical Therapist to assist residents in achieving their highest level of functional independence, mobility, and physical well-being.

Essential Duties and Responsibilities

• Assist the Physical Therapist in implementing individualized care plans and treatment programs.

• Provide therapeutic exercises, ambulation, balance training, and other physical therapy interventions as prescribed.

• Observe, document, and report residents’ progress, responses, or changes in condition to the Physical Therapist.

• Educate residents and families on exercises, mobility techniques, and safety practices to promote independence.

• Maintain accurate, timely, and thorough documentation in accordance with facility and regulatory standards.

• Ensure proper use and maintenance of therapy equipment and supplies.

• Promote resident engagement and a positive therapeutic experience.

• Work collaboratively with nursing, occupational therapy, and restorative teams to ensure continuity of care.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program .

• Current, valid Maryland PTA license or eligibility for licensure.

• Strong communication and interpersonal skills.

• Compassionate and professional demeanor.

• Ability to lift/move up to 50 lbs and assist with resident transfers.

• Excellent time management and attention to detail.

Preferred: Experience in a skilled nursing or long-term care setting.
