Provide case management to children in Patrick County local schools (Woolwine and Meadows of Dan). Position is responsible for ensuring that youth receive services to assist them in accessing the needed medical, psychiatric, social, education and other supports essential to meeting their basic needs. Qualifications: Bachelor's Degree in human services or related field is preferred with one year experience of direct care or any equivalent combination of training, skills and abilities as defined by DMAS. Some flexibility in schedules may be an option. Salary is commensurate with education & experience as it relates to the position.
Must be willing to consent to background checks, including checks for barrier crimes, and drug screening as part of the application process.
