Job Description
Physical Therapist - Home Health

Schedule

Day shift, 5 days a week, 8-hour shifts from 08:00 to 16:30.

Assignment Details

Full-time position with a productivity expectation of around 25 units for 7 visits per week.

Requirements

• Minimum of 1 year of experience; first-time travelers accepted.
• Active BLS certification.
• Georgia Physical Therapy license or compact Physical Therapy license accepted (verification required).
• Experience in home health with a focus on geriatrics, strokes, and orthopedics preferred.
• Familiarity with Home Care Home Base (HCHB) preferred; if not, completion of an online HCHB course is required (cost $250, must be paid upfront by traveler).

Responsibilities

• Provide physical therapy services to adult and geriatric patients in a home health setting.
• Assess patient conditions and develop individualized treatment plans.
• Monitor patient progress and adjust treatments as necessary.
• Educate patients and families on rehabilitation strategies and exercises.
• Document patient care accurately and timely.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.

Compliance / Notes

Full compliance required to start on time; non-compliance may lead to a two-week delay due to orientation scheduling. If holidays occur during the assignment period, the traveler may be required to work them as specified.
