Physical Therapist

Schedule: 8 hours

Assignment Details:

• Contract Length: 14 weeks
• Guaranteed Hours: 40

Requirements:

• Current State License as a Physical Therapist
• Proof of current CPR certification
• Current driver's license. Must have reliable transportation and insurance
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an accredited program

Roles & Responsibilities:

• Conduct clinical assessments to determine patient needs and treatment appropriateness.
• Document and maintain comprehensive patient care plans and progress notes; revise plans based on ongoing assessments.
• Provide treatment for neuromuscular, neurological, and orthopedic disorders according to medical referrals.
• Educate patients and families on therapeutic procedures and correct equipment usage.
• Supervise other team members in providing care consistent with patient needs and desires.
• Apply infection control measures during patient care activities.
