We are currently hiring for a Hospice Field Staff LPN position that would be based out of our Belleville, IL office. Heres a short video about working for Hospice of Southern Illinois: Position Summary Under the supervision of a Registered Nurse, and in collaboration with the Interdisciplinary Team (IDT), provides direct and indirect patient care in accordance with the Nurse Practice Act and organizational policy. Pay Pay : $28.72 Hourly. Full-Time Non-Exempt Position. Schedule Schedule : Monday- Friday 8:30 AM- 5:00 PM with an additional rotational on-call schedule that includes some evening, weekends, and holidays. As we have on-call nurses, on-call would be limited. Location Location : Will be based out of our Belleville Patient Services Office. Would primarily visit patients and travel within the Belleville offices service area northern service territory, which include the following counties: Clinton, Madison, Bond, Montgomery, Macoupin, Jersey, and Calhoun. We visit our patients wherever they may be located, whether it be in their homes, long-term care facilities, nursing homes, hospitals, etc. As a result, a valid driver's license, current automobile insurance, and reliable transportation are requirements of the position. The company reimburses .70 cents per mile for driving personal vehicles. EOE Essential Functions of the Field Staff LPN Position Positively collaborates in the development and modification of the Field Staff Nurse/Case Manager comprehensive nursing plan of care for the hospice patient. Performs physical assessment and collects data then effectively collaborates with the Field Staff Nurse/Case Manager in the assessment of the health status of a patient. Implements aspects of the plan of care, as delegated. Participates in health teaching and counseling to promote, attain, and maintain the optimum health level of patients, as delegated. Serves as an advocate for the patient/family/caregiver by communicating and collaborating with the IDT, as delegated. Participates in the evaluation of patient responses to interventions. Provides direct patient care. Participates in on-call after-hour care as needed and in the absence of assigned on-call staff. In the on-call role, the LPN triages calls and provides care under the direction of a Registered Nurse. Demonstrates basic LPN clinical skills. Gives a practical exhibition and explanation at the required annual skills day. Knowledgeable in patterns of disease progression at the end-of-life and provides effective, ongoing patient/family/caregiver education, as delegated. Practices within the guidelines of the Illinois Department of Professional Regulation. Provides evidence-based physical, emotional, psychosocial, and spiritual care under the direction of a Registered Nurse.
