Position: Home Health Occupational Therapist (OT)

Location: Fremont, CA; Castro Valley, CA; Ashland, CA; Cherryland, CA; Hayward, CA; Newark, CA; San Leandro, CA; San Lorenzo, CA; Union City, CA; Oakland, CA; Berkeley, CA; Alameda, CA; Albany, CA; Emeryville, CA; Piedmont, CA

Shift Details:

• Type: Day Shift
• Schedule: 5x8-Hour (08:00 AM - 04:30 PM)
• Guaranteed Hours: 36 hours per week
• Patient Visits: Expected to see 5-6 patients per day

Start Date: ASAP (Compliance may take about 2 weeks)

Job Responsibilities:

• Deliver occupational therapy services to patients in their homes.
• Manage a caseload of home health patients while adhering to organizational productivity standards.
• Maintain timely communication and accurate documentation of patient progress and care.
• Utilize provided documentation tools effectively and return them upon completion of assignment.

Requirements:

• 1 year of preferred home health experience.
• Must hold a valid California State Occupational Therapist license.
• BLS certification required.
• Possess reliable transportation, with occasional travel of up to 50 miles between patient visits.

Additional Information:

• Weekend patient visits are optional to help meet productivity goals.
• No on-call duties required.
• Locals and first-time travelers are welcome to apply.
• Any color scrubs are acceptable for attire.
• Free parking is available on-site.
