Seven Healthcare are seeking a skilled Physical Therapist to join the Finger Lakes team, specializing in home health. This is a travel/contract position where you will provide case management and direct patient care in a supportive team environment.

Key Responsibilities of the Physical Therapist
- Provide case management and direct patient care
- Administer physical therapy modalities in a home setting
- Assist the physician in evaluating level of function and develop plan of care
- Prepare clinical and progress notes
- Consult with families and agency personnel
- Participate in in-service programs
Requirements of the Physical Therapist
- B.S., M.S. or D.P.T. degree in Physical Therapy
- Possession of a current New York State License in Physical Therapy
- BLS (AHA ONLY)
- Minimum of one year experience in a Long Term Care/Home Care setting as a Physical Therapist
- Recent Home Health Experience within the last year
- Oasis experience is required
