We're seeking a dedicated **Physical Therapist** to join our outstanding rehabilitation team on a **Part-Time or Per Diem** basis.Whether you're looking for flexibility, supplemental income, or an opportunity to make a meaningful impact, we'd love to meet you.#### What You'll Do* Evaluate residents and develop individualized treatment plans* Provide skilled physical therapy services to post-acute and long-term care residents* Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families* Monitor patient progress and modify treatment plans as needed* Maintain accurate and timely documentation* Educate residents and caregivers on exercises, safety, mobility, and discharge recommendations* Help residents achieve their highest level of independence and quality of life#### Qualifications### Required:* Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program* Current California Physical Therapist license in good standing* Strong clinical assessment and treatment skills* Excellent communication and teamwork abilities###### Preferred:* Experience in Skilled Nursing, Post-Acute, Rehabilitation, or Long-Term Care settings* Knowledge of Medicare documentation and reimbursement guidelines* Experience treating orthopedic, neurological, and medically complex patients##* Competitive pay: **$65-$70/hour*** Flexible Part-Time and Per Diem scheduling* Supportive interdisciplinary team* Modern rehabilitation environment* Opportunities to work with a diverse patient population* Leadership team committed to clinical excellence and employee success#### The Ideal CandidateYou are compassionate, patient-focused, and motivated to help individuals recover from illness, injury, or surgery.
