Location First Choice Immediate Care – McCreary, Whitley City, KY 42653 Position Full Time – LCSW / LPCC / LMFT Responsibilities Provides comprehensive psychosocial assessments and behavioral health care. Uses age‑appropriate assessment and intervention skills and a range of interventions based on patient needs. Assists members in developing cost‑effective, appropriate health care plans by optimizing personal and community resources. Promotes multi‑disciplinary, patient‑centered care that considers personal, psychological, economic, and cultural needs. Maintains thorough knowledge of CFMC services, community resources, and applicable federal/state laws affecting health care planning. Completes required documentation of services and plans of care per policy. Participates in program development, orientation, and educational activities to build members' capacity for self‑directed health care. Other duties as assigned. Qualifications One (1) year of experience in mental health, hospital, medical outpatient clinic, FQHC, home health, or long‑term care setting. Three to five (3‑5) years of experience in a related environment. Master’s degree or equivalent with appropriate licenses, certifications, and registrations. Current unrestricted license in the state of practice, required upon hire. Basic Life Support (BLS) for Health Care Providers. National Provider Identifier (NPI) required prior to employment start date. Key Skills Expertise with brief counseling interventions. Demonstrated ability to provide leadership. Advanced knowledge of local and national resources related to behavioral health care. Schedule Requirements Full‑time position, five days per week, 100% clinic‑based, in‑person work required. Work schedules are set at the discretion of CFMC and may be adjusted at any time based on operational needs. Compensation Negotiable based on experience.
