We're seeking a Registered Nurse (RN) Clinical Manager to oversee clinical operations and patient care services in our home health division. Key Responsibilities

• Provide daily clinical oversight of patient care services.
• Coordinate referrals and admissions with internal and external sources.
• Evaluate patient needs and agency capabilities before admission.
• Supervise care planning, partner assignments, and care delivery.
• Ensure compliance with regulatory standards and agency policies.
• Mentor and coach clinical staff to promote morale and retention.
• Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.

Qualifications

Required: Active, unrestricted RN license in the state of practice. Minimum of two years RN experience (preferred). At least one year of Home Health experience (preferred). Current CPR certification.
