Registered NurseThe registered nurse is expected to provide direction to members of the patient care team through case management of a patient. Case management of patient services includes:• Perform initial evaluation, follow up visit and discharge of your patient.• Establish the patient's individualized treatment (Plan of Care) and medical record baseline.• Collaborate with physicians, other agency staff and contract services to coordinate and implement the Plan of Care.• Trains new clinicians by allowing clinicians to shadow patient visits.• Documents direct patient services, coordination and collaboration with physician and other disciplines.• Promotes the agency and services to the public and participate in physician and community education related to services provided by the agency as registered nurse.QualificationRequired:• Current California Registered Nurse license• Able to travel to prospective patient's place of residence• Able to stoop and bend; must be able to lift and transfer patientsDesired:• Experience with a public health, home care, home health care, or hospital background• Ability to demonstrate communication, presentation, interpersonal, analytical and problem-solving skills• Bilingual
