Job Description This part time position (24 hours including weekends) is responsible for managing the residents’ health plan and monitoring the day‑to‑day nursing activities during the shift in such a way that meets the needs of the residents, complies with the Home Health Program policy and procedures and with the mission, goals and policies of Avinity. Qualifications Current Minnesota Licensure as a Licensed Practical Nurse. 2 years experience in home care or assisted living required. 1 year experience in monitoring HHAs preferred. Must meet and maintain the requirements of the Department of Human Services background check for health providers. Must meet and maintain the requirements of Avinity’s criminal and drug testing background check policies. Must meet and maintain the physical requirements of the position. Understand and communicate with specialized medical terminology and the ability to explain procedures to residents, family members.
