Job Summary

Le'Fevre Personal Care, Inc. is seeking a compassionate, dependable Licensed Practical Nurse (LPN) to join our clinical team as an Independent Contractor. This position provides flexible scheduling and is ideal for an experienced LPN looking to supplement their income while making a meaningful impact in the lives of individuals with developmental disabilities.

Working under the supervision of the Registered Nurse (RN), the LPN will conduct nursing assessments, supervisory visits, and documentation while ensuring quality care and compliance with DBHDD and company standards.

• Conduct initial nursing assessments in clients' homes.
• Collect assessment data and complete required documentation.
• Assist with development of individualized care plans.
• Conduct required 90-day supervisory visits.
• Monitor client health and report changes to the supervising RN.
• Evaluate staff performance and adherence to care plans during supervisory visits.
• Complete all documentation accurately within 24 hours.
• Maintain compliance with HIPAA, DBHDD regulations, and company policies.
• Communicate effectively with the RN, management team, caregivers, and families.

Note: The supervising RN reviews, approves, and signs all assessments and care plans.

Required

• Active Georgia Licensed Practical Nurse (LPN) license
• Current CPR Certification
• Current TB Screening/Health Clearance
• Valid Driver's License
• Reliable transportation
• Ability to travel throughout Paulding and Cobb Counties
• Ability to pass a criminal background check

Preferred

• Experience working with individuals with intellectual and developmental disabilities (I/DD)
• Excellent communication and documentation skills
• Ability to work independently while collaborating with the RN and management team

• Flexible schedule
• Weekday visits preferred
• Contract work based on assigned client visits

Pay

• $100 per Initial Nursing Assessment
• $65 per 90-Day Supervisory Visit
• $30/hour for pre-approved extended visits
