Transition Coordinator

Transition Coordinator Community Bridges, Inc. (CBI) is an integrated behavioral health agency offering residential, outpatient, inpatient, medical home, medication‑assisted treatment, and crisis services across Arizona. The Transition Coordinator supports patients at each level of care to ensure a seamless discharge and ongoing treatment plan. Job Summary Transition Coordinators are responsible for developing individualized discharge plans, scheduling follow‑up appointments within seven days of discharge, coordinating authorizations through Utilization Management, and completing initial assessments and treatment plans. They provide therapeutic services—including counseling, group work, and discharge planning—to patients and their support systems. Responsibilities Develop individualized discharge plans for patients. Schedule all follow‑up appointments for ongoing care within 7 days of discharge. Coordinate with Utilization Management on all authorizations and continued authorizations. Complete initial assessment and treatment plan for each patient. Provide therapeutic services, including counseling, group work, discharge planning, and authorization coordination. Skills & Requirements High school diploma or GED required; associate’s degree in a behavioral health related field preferred. Minimum of 2 years of full‑time health‑care related work experience with exposure to behavioral and medical situations.
