Job Description

Your impact:

• Deliver field-based care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient's home.
• Manage chronic disease: Create longitudinal care plans that move the needle for high-risk patients.
• Collaborate across the continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
• Prevent ER visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.

Job Requirements

What you bring:

• Advanced degree: Master's Degree (NP) with a preferred focus on Adult, Acute Care, Geriatric, and/or Family.
• State required credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP).
• Reliable mobility: Valid driver's license and vehicle for local field-based travel.
