Registered Nurse – Home Care (Full-Time)

A leading home healthcare organization is seeking a Registered Nurse to provide comprehensive nursing care to patients in their homes throughout Monmouth County. This role offers the opportunity to build meaningful one-on-one relationships with patients while delivering high-quality clinical care in a home-based setting.

The Registered Nurse will be responsible for assessing patient needs, developing and implementing care plans, coordinating services with interdisciplinary team members, and ensuring compliance with regulatory and physician-directed care requirements.

Key responsibilities include conducting patient assessments and developing individualized care plans, delivering skilled nursing care in accordance with physician orders, agency policies, and professional standards, monitoring patient progress and adjusting care plans as needed to support optimal outcomes, completing medication reconciliation and collaborating with physicians regarding treatment plans, ensuring accurate, timely, and compliant clinical documentation, educating patients and caregivers using appropriate teaching methods and documenting educational interventions, coordinating care with physicians, case managers, therapists, home health aides, and other healthcare professionals, monitoring authorization requirements and maintaining compliance with payer guidelines, reporting patient concerns, changes in condition, and care issues promptly to appropriate personnel, supervising home health aides and completing required supervisory visits within regulatory guidelines, participating in patient care transitions and communicating care plans effectively among team members, maintaining productivity expectations and completing assignments independently and accurately, staying current with nursing standards, clinical guidelines, and best practices, and performing additional duties as assigned.

Required qualifications include an associate degree in nursing (ADN) or diploma in nursing, minimum of six (6) months of registered nurse experience or at least one (1) year of licensed practical nurse experience, experience in home health or hospice care preferred, basic computer proficiency and documentation skills, active New Jersey registered nurse (RN) license, and current AHA basic life support (BLS) for healthcare providers certification.

Preferred qualifications include a bachelor of science in nursing (BSN), prior home health nursing experience, and strong experience in care coordination and patient education.
