Telligen is seeking a Care Manager in Illinois to coordinate person-centered care for members with complex medical, behavioral health, functional, and social needs. You will complete assessments, develop and monitor care plans, and connect members with healthcare and community resources, guiding transitions to community living with dignity and independence.
