Patient Care Coordinator

Job Description

The Patient Care Coordinator is the hub of our scheduling and follow-through process. You will coordinate appointments across our imaging facilities, confirm attendance, and proactively follow up to ensure that services are rendered and reports are received on time. This role requires bilingual fluency in English and Spanish , as well as excellent multitasking skills in a fast-paced, detail-oriented environment.

Responsibilities

  • Receive referrals; verify case details, attorney contact, and LOP/authorization status.
  • Schedule and confirm patient appointments with contracted facilities; coordinate transportation when needed.
  • Monitor daily schedules; manage reminders, reschedules, and no-shows; document all touchpoints.
  • Follow up with facilities to verify services were completed; obtain imaging, procedure notes, and billing packets.
  • Maintain accurate records in EMR/CRM, trackers, and shared logs; escalate barriers promptly.
  • Communicate clearly with patients (English/Spanish), provider offices, and law firms to keep cases moving.
  • Track and report on KPIs (e.g., time-to-schedule, scheduled-to-seen conversion, no-show rate, report turnaround).
  • Protect PHI and uphold HIPAA compliance at all times; follow policies and SOPs.

Qualifications

  • Bilingual: English & Spanish (required).
  • 1–3+ years in medical scheduling, care coordination, or PI/med-legal admin preferred.
  • Working knowledge of PI workflows (LOP, attorney communication, records retrieval) is a plus.
  • Proficient with EMR/CRM systems, e-fax, and Microsoft 365/Google Workspace; strong phone etiquette.
  • Exceptional organization, multitasking, and attention to detail in a high-volume setting.
  • Professional, empathetic communication with patients and partners.
