Hospice RN Eval / Admission Nurse (10:00 AM – 7:00 PM)

Choice Hospice is seeking an experienced Hospice RN Evaluator/Admission Nurse to provide compassionate, patient-centered care to individuals with life-limiting illnesses. In this role, you will complete hospice evaluations and admissions, determine eligibility, develop individualized plans of care, and educate patients and families while ensuring a smooth transition into hospice services.

What You'll Do

• Complete hospice evaluations, admissions, and comprehensive nursing assessments.
• Determine hospice eligibility using admission and recertification criteria.
• Develop, implement, and update individualized plans of care.
• Coordinate care with physicians and the interdisciplinary hospice team.
• Educate patients and families on disease progression, hospice philosophy, medications, symptom management, nutrition, and end-of-life care.
• Assess physical, emotional, psychosocial, spiritual, and environmental needs.
• Evaluate caregiver readiness and provide ongoing support and education.
• Supervise and provide clinical direction to Hospice Aides and LPN/LVNs.
• Document assessments, admissions, care plans, and clinical progress accurately and timely.
• Communicate changes in patient condition to the physician and interdisciplinary team.
• Participate in case conferences and quality improvement initiatives.
• Perform on-call responsibilities as assigned.

Qualifications

• Current Registered Nurse (RN) license.
• Current CPR certification.
• Valid driver's license and reliable transportation with current auto insurance.
• Minimum of 2 years of hospice nursing experience preferred.
• HomeCare HomeBase experience preferred.
• Excellent assessment, communication, and critical thinking skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
