MDS Coordinator
Job Summary:
We are seeking a highly organized and detail-oriented professional to join our team and lead our clinical assessment and care planning efforts.
Responsibilities:
Coordinate and facilitate the development of the Minimum Data Set (MDS) for each resident, ensuring accuracy, completeness, and compliance with regulations
Collaborate with the interdisciplinary team to identify resident needs and develop individualized care plans
Conduct thorough assessments and evaluations to identify resident goals and outcomes
Maintain accurate and detailed records of resident assessments, care plans, and progress
Develop and implement strategies to improve quality of care and resident outcomes
Participate in facility-wide quality improvement initiatives and projects
Maintain knowledge of relevant regulations, policies, and procedures
