RN Registered Nurse

Overview STGi is currently seeking a Registered Nurse (RN) to provide services at our Community Based Outpatient Clinic (CBOC) in Plattsburgh, New York. The RN Care Manager represents the interests of Veteran patients and coordinates care with VA medical centers and stakeholders. The RN provides professional clinical services under the direction of the CBOC and manages patient care. Responsibilities Provide nursing care based on orders from physicians, mid-level providers, and the CBOC Clinic Manager to meet the physical, mental, and emotional needs of patients. Supervise support staff in delivering care to patients. Lead and participate in PACT team huddles. Collaborate with the Primary Care Physician to ensure adequate staffing and promote continuity of care between the VAMC and the clinic. Maintain universal precautions and infection control practices. Supervise medical assistants and/or licensed practical nurses who obtain initial patient information (vital signs, symptoms, etc.). Prepare patients for examination and assist the PACT Provider as requested. Ensure smooth and timely flow of patients according to VA access standards and triage protocol. Perform triage activities for walk-in patients and medical phone inquiries. Administer medications per facility protocols as prescribed by the provider. Perform CPR and assist during respiratory and cardiac arrest procedures. Perform nursing procedures including dressing changes, suture removal, oxygen administration, wound care, catheterizations, and venipunctures. Prepare patients for laboratory procedures and provide information/instructions. Set up and operate therapeutic and emergency equipment (monitor, ECG, oxygen, suction). Document patient information in medical records and VISTA to demonstrate quality care and continuity. Keep the VISTA clinic reminder system current for each enrolled patient. Assist in scheduling patients for initial and follow-up appointments according to VA access standards and medical necessity. Keep patients informed about their plan of care and test results as needed. Provide health materials, resources, and education to patients and families as needed. Assist providers in ensuring all patient information in electronic encounter documentation is complete and accurate. Participate in the ongoing Performance Improvement Program with the VAMC subcontractors. Ensure all required reports and clinical/administrative documentation are completed accurately and timely. Participate in day-to-day CBOC operations management. Comply with federal, state, local, OSHA, VA, subcontractor safety and regulatory requirements. Maintain confidentiality and patient privacy, rights, and safety. Perform the duties of the CBOC Clinic Manager when absent (if applicable). Perform other work-related duties as assigned. Required Skills and Qualifications Three years of experience in Medical-Surgical Unit, Outpatient Facility, Primary Care, and/or Adult Health preferred. Current NY nursing license (unrestricted). Current CPR/BLS certification from AHA or MTN. Knowledge of professional nursing principles, procedures, and techniques. Ability to assess patient condition, recognize adverse signs, and react swiftly in emergencies. Ability to set realistic, measurable education goals for patients and families. Ability to provide individualized and group counseling, guidance, and health instruction focused on disease prevention and health maintenance. Ability to teach patients and families proper home health care activities (e.g., injections, taking vitals, wound care). Knowledge of pharmacology and medications used in patient treatment, including effects, side effects, and considerations. Strong communication skills and customer service experience; ability to handle difficult situations professionally. Proficiency with Microsoft Office, Electronic Medical Records, and computer skills. Attention to detail and ability to multi-task. Required Education and Working Conditions RN degree from an approved nursing program; ACEN or CCNE accreditation required. BSN preferred. Working conditions: moderate physical activity; up to 25 pounds; standing and walking for more than four hours per day; typical interior/office environment. Respond to medical emergencies within the clinic as needed. Compensation and Equal Opportunity Pay rate: $33/hour.
