Pediatric Advanced Practice Nurse / Physician Assistant - Gastroenterology

Overview Nemours Children's Health is seeking an experienced full-time Advanced Practice Provider for our Jacksonville GI Team. Responsibilities Practices collaboratively with physicians and other health care providers to provide high quality healthcare to children. Performs appropriate assessment of patient/family upon referral/admission. Performs ongoing assessment and evaluation of patient progress and revision of interdisciplinary plan of care. May use prescriptive authority if appropriately credentialed to do so. Collaborates interprofessionally as needed. Identifies patient/family need for further resources and initiates appropriate referrals. Assists the Staff Nurse in formulating the Nursing Care Plan and setting priorities based on the needs of patient and family. Documents patient care and progress on patient\'s chart. Coordinates Nursing Plan for Discharge in collaboration with the Staff Nurse and other members of the health care team.
