Your Role

• Provide comprehensive primarycare to older adult patients
• Take ownership of outcomes whiledelivering personalized, relationship-driven care
• Serve as a health coach ,guiding patients toward sustainable lifestyle changes
• Coordinate specialty and hospitalcare while maintaining continuity
• Lead and collaborate with yourcare team
• Supervise and support AdvancedPractice Providers (APPs)
• Partner with leadership onquality, experience, and cost goals

What You Bring

• MD or DO (BoardEligible/Certified in Internal Medicine, Family Medicine, or Geriatrics)
• Active (or eligible) Virginiamedical license & DEA
• Passion for value-based,patient-centered care
• Collaborative, team-orientedmindset

Compensation & Benefits

• $250,000 - $306,714 base salary (based on experience)
