Position Highlights:

• One-on-one patient care

Responsibilities:

• Providing skilled nursing care in the home setting
• Medication administration and treatments
• Monitoring and documenting patient condition
• Following individualized care plans
• Collaborating with physicians, families, and care teams
• Ensuring patient comfort, safety, and quality of care

Requirements:

• Current Texas Lvn license in good standing
• Strong clinical and communication skills
• Compassionate, dependable, and professional attitude
• Home health or private duty experience preferred
• Current Cpr certification preferred
• Reliable transportation
