Evening Home Care LPN (4 PM–Midnight)

As an Evening Home Care LPN, you'll provide skilled nursing care, comfort, and support to hospice patients and families in their homes during evening hours. This role is ideal for an LPN who enjoys working independently, building meaningful relationships, and making a real difference in the lives of others.

Schedule: Monday–Friday, 4:00 PM–Midnight

Responsibilities

• Provide skilled nursing care under the direction of a Registered Nurse.
• Monitor patient conditions and communicate changes to the care team.
• Administer medications and treatments as ordered.
• Support patients and families through education and guidance.
• Document care accurately in the electronic medical record.
• Collaborate with the interdisciplinary team to ensure exceptional patient care.

Qualifications

• Current LPN license in Virginia.
• One year of nursing experience preferred.
• Hospice, home health, or long-term care experience preferred.
• Strong communication and critical thinking skills.
• Valid driver's license and reliable transportation.
