Job Description

We are seeking a skilled Home Health Physical Therapist Assistant to provide care in the West Portland area and surrounding regions. The role involves working closely with patients in their homes to facilitate recovery and enhance mobility.

Responsibilities

• Collaborate with healthcare teams to develop and implement patient-specific treatment plans.
• Help patients perform therapeutic exercises and activities of daily living.
• Educate patients and families on care plans and progress.
• Maintain accurate documentation and patient records using EPIC Remote Client and Rover.
• Manage a caseload efficiently, seeing up to 6 patients per day.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• Must possess a current BCLS certification.
• Reliable transportation is required as patient visits may extend up to 60 miles.
• COVID-19 vaccination series with one booster is required.
