Nurse Practitioner or Physician Assistant - PRN

We serve older adults and other at‑risk individuals across the nation by meeting members wherever they are. Responsibilities Conduct adult or geriatric assessments, including medical history, diagnosis and treatment, health education, physician referrals, case‑management referrals, follow‑up, and clear documentation according to Matrix guidelines and protocols. Administer and order point‑of‑care and lab‑based testing as needed. Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate. Collaborate with a Primary Care Physician on patient education and follow‑up as necessary. Provide services at the member’s place of residence, which may be their home or a Skilled Nursing Facility. Compensation & Benefits Flexibility: Visits are scheduled based on your availability. Sessions run Sunday‑Saturday from 8 am to 7 pm, with options for day, evening, or weekend work. Competitive Pay: Earn income based on the number of assessments completed. Monthly earnings range from $1,800 to $11,500 (20–100 visits) with a per‑assessment rate of $90–$115 depending on state. Current NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority as needed. AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty. Current BLS, ACLS or CPR certification. One year of NP experience preferred; new graduates encouraged to apply. Valid state driver’s license for the state of residence. Automobile insurance coverage required for the state of residence (or ability to obtain). Insurance requirement: Bodily Injury – $100,000 per person/$300,000 per incident; Property Damage – $50,000; Collision and comprehensive deductibles – $1,000 or less. Strong computer skills and familiarity with employee health/medical record software. Excellent verbal and written communication skills.
