Medical-Surgical Registered Nurse (Med-Surg RN)

Overview Yale New Haven Hospital, Saint Raphael Campus is actively hiring Registered Nurses for the following Medicine units: SLA 3 – Adult Acute Care Medicine with Telemetry SLA 4 – Adult Acute Care Medicine with Telemetry SLA 5 – Adult Acute Care Renal and Palliative Specialty Medicine V5N V5W – Adult Acute Care Medicine with Telemetry V4W – Adult Acute Care Medicine with Telemetry Acute Care for Older Adults Medicine with Telemetry Cel 3 Acute Care Medicine Cel 4 Acute Care Medicine Responsibilities Work under the supervision of the Nurse Manager/Clinical Coordinator. Assess, plan, implement, evaluate, and supervise individual patient care on a nursing unit/department according to unit policies. Provide supervision of non‑RN personnel on the assigned unit. When assuming the charge role, delegate and supervise unit activities. May be required to work on other nursing units according to staff and patient distribution. Perform other duties as assigned or requested to maintain a high level of service. Assess the patient’s physical and psychosocial condition on an ongoing basis throughout the continuum of care. Develop, communicate, and document a plan of care reflecting individualized patient problems and interventions to achieve desired outcomes, involving the patient, family, and healthcare team. Implement and coordinate this plan of care in collaboration with the healthcare team. Evaluate the effectiveness of the plan of care, patient response to care, and readiness for discharge; revise as needed. Make referrals where indicated. Plan for education after assessing the individual learning needs and readiness of the patient and family. Provide educational opportunities and document outcomes. Serve as a preceptor to new staff and as a charge nurse. Meet performance expectations for customer service, teamwork, resource utilization, and staff/self‑development as outlined in the performance review. Demonstrate competency in the care of the neuro patient, including skill and knowledge in neuro assessment, airway management of the neuro patient, care of a trached patient, assistance during special neuro procedures, and use of equipment and care of the patient in a halo brace. Perform pharyngeal, endotracheal, tracheostomy suctioning according to established policy and procedure. Perform 'Tia' teaching program duties. Maintain a high level of supervisory and clinical competence across duties. Qualifications Graduate of an accredited school of nursing; BSN preferred. Proficient in use of computer software, including electronic patient records. Ability to use the internet for literature searches regarding patient care. Valid State of Connecticut Registered Nurse license. BLS certification required. This position is an equal opportunity employer.
