The primary responsibility of the Community Dental Program Coordinator is to organize mobile and school-based dentistry events and support AMH patients by connecting them with dental, medical, and community resources that promote access to care, self-sufficiency, and self-efficacy.

The Community Dental Program Coordinator will coordinate events and partnerships associated with the Community Dental Program, school-based oral health program, mobile dentistry, and hospital operating room/surgical center. The Coordinator will assist patients in accessing public benefits such as Medicaid and other community resources and will support patients in navigating care within AMH, including referrals to medical and dental services.

A key responsibility of this position is to coordinate and facilitate patient scheduling for dental procedures requiring care at the operating room (OR) or surgical center, including coordinating with patients, AMH providers, hospital or surgical center staff, and other involved parties to ensure cases are appropriately scheduled and prepared. The Coordinator will also track and follow up on dental and medical referrals to help ensure patients successfully connect with referred providers and that referral status and outcomes are appropriately documented and communicated to the dental team.

The Coordinator will maintain professional and respectful relationships with community partners in order to maintain an up-to-date database of active community resources and ensure AMH patients are appropriately connected to available services.

Background check and drug screen required.

KEY ACCOUNTABILITIES:

• Coordinate events with community partners and outside organizations in support of the broader Community Dental Program.
• Coordinate and organize mobile and school-based dental events, including communication and scheduling with schools and participating pre-K and organizations.
• Support patients in accessing care resources, including referrals to dental specialists, behavioral health programming, medical care, and other appropriate services.
• Coordinate patient scheduling and logistics for dental procedures requiring the operating room (OR) or surgical center, including communication with patients, AMH providers, hospital/surgical center staff, and other involved parties.
• Track OR and surgical center cases from referral through scheduling and completion, ensuring required information, documentation, appointments, and follow-up activities are addressed in a timely manner.
• Follow up on dental, medical, and specialty referrals to determine whether patients have connected with the referred provider and to identify and address barriers that may prevent completion of recommended care.
• Maintain accurate referral tracking and communicate referral status, scheduling information, and relevant updates to AMH providers and appropriate members of the care team.
• Navigate patients through the health system, with an emphasis on dental care and continuity of care.
• Assist patients in accessing community resources, including Medicaid applications and other public benefits and services.
• Maintain an up-to-date Community Partner and Resource database and maintain collaborative relationships with community partners to ensure AMH patients are well represented and connected to appropriate resources within the region.
• Collaborate with AMH teammates to provide excellent wraparound services that enhance patient care and improve access to services.
• Assist with data collection and reporting for quality improvement and quality assurance measures.
• Maintain timely and accurate documentation of patient navigation, referrals, scheduling activities, and follow-up in the Electronic Health Record and other applicable systems.
• Adhere to AMH’s policies and procedures and support an environment based on mutual respect and ethical communication with staff, patients, families, and community partners.
• Attend all mandatory AMH meetings and Dental Team meetings.

KNOWLEDGE & SKILLS

• Experience and knowledge of general and public health dentistry.
• Excellent interpersonal communication, organizational, and community-building skills.
• Understanding of community resources, public benefits, and healthcare navigation.
• Ability to coordinate complex patient scheduling and follow-up involving multiple providers, patients, healthcare facilities, and community partners.
• Strong follow-through and referral-tracking skills, with the ability to monitor referrals and scheduling needs through completion and communicate status appropriately.
• Skilled at time management; well organized; able to prioritize multiple responsibilities, meet deadlines, and respond to staff and patient requests in a timely manner.
• Proficient with technology, with experience documenting in Electronic Health Records.
• Ability to work within a healthcare setting and relate well to people from diverse ethnic and cultural backgrounds, as well as a passion for working with at-risk and culturally diverse populations.
• Ability to read, understand, and follow program guidelines and service materials.
• Willingness to work within the clinic schedule and be available during hours of operation.
• Openness to accepting feedback and willingness to adjust where necessary for program adherence, staff safety, and patient safety.
• Strict adherence to all Federal and State regulations as they apply to healthcare, health information, patient care, and documentation of patient care.
