Physical Therapist - Home Health Location Montgomery County, MD Assignment Duration This is a 13-week contract position. Schedule Day shift, Monday to Friday (08:00 - 17:00). Job Description We are seeking a dedicated and skilled Physical Therapist to join our home health team. The ideal candidate will be responsible for providing high-quality care to adult patients in their homes. This position requires a compassionate approach and the ability to work independently while ensuring the well-being of our patients. Responsibilities Conduct patient assessments and evaluations to determine care needs. Develop and implement personalized treatment plans. Perform therapy interventions to improve patient mobility and functionality. Educate patients and their families about home exercise programs and therapy techniques. Maintain accurate and timely documentation of patient progress and therapy outcomes. Collaborate with interdisciplinary teams to optimize patient care. Required Experience / Certifications / Licensure Valid Maryland PT State License required. Current BLS certification is mandatory. Minimum of 1 year of experience in a related field, preferably in home health. Experience with adult health care and patient management is essential. Two supervisor references required.
