Contract Details

• Position Type: Contract
• Contract Duration: 3 months
• Pay Rate: $75.00 / Hour (USD)
• Shift / Schedule: Day (5x8 Hours), 8:00-16:30
• Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks)
• Travel Requirements: Traveler is expected to have their own transportation. Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day. System does a good job grouping clients minimizing drive and windshield time.
• Work Authorization: Applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.

Job Summary

This is a Home Health Physical Therapist position covering West Portland, OR and surrounding areas. Open to PTA, if necessary. The position is an ASAP start with 8-hour day shifts. Ratios are 1:1, with visits of up to 6 patients per day and a 30-productivity-points standard. First-time travelers are accepted, and local candidates within a 50-75 mile radius are accepted at the same rate.

Key Responsibilities

• Provide skilled home health physical therapy services to patients in West Portland, OR and surrounding areas
• Visit up to 6 patients per day at a 30-productivity-points standard
• Chart using EPIC Remote Client for Home Health (on laptop) and Rover (phone app)
• Maintain 1:1 patient ratios throughout the course of care

Required Skills

• Minimum 1 year of skilled home health experience required
• Valid PT License (Required)
• BLS/BCLS certification required
• Must have own reliable transportation
• Ability to manage a patient caseload with geographic distribution across a service area with an occasional maximum radius of roughly 60 miles per day

Education Requirements

• PT License required
• BCLS (Basic Cardiac Life Support) certification required
• Pending license accepted

Required Experience

• Minimum 1 year of skilled home health experience required

• Charting: EPIC Remote Client for Home Health (on laptop) and Rover (phone app)
• Parking Cost: Free
• Mileage Reimbursement: IRS Standard
• No on-call required
• No weekend requirement
• Block schedule cannot be accommodated (no 4x10s or 3x12s)
• Will not float between like units
• Facility requires primary series COVID vaccination in addition to one booster
