REGISTERED NURSE (RN) Home Health & Hospice
P&A RECRUITING LLC
Description Perform patient assessments and ongoing evaluations Develop and update individualized plans of care Provide skilled nursing services in the patient's home Monitor patient conditions and report significant changes Administer medications and treatments as ordered Provide wound care and other skilled nursing services as needed Coordinate care with physicians, hospice/home health teams, caregivers, and families Educate patients and families regarding care and medications Complete accurate and timely clinical documentation Provide compassionate end-of-life care for hospice patients Requirements Current, unrestricted Texas RN license Current CPR/BLS certification Valid driver's license and reliable transportation Ability to travel to patients' homes Strong communication and documentation skills Home Health and/or Hospice experience preferred Must successfully complete required background screening and onboarding #J-18808-Ljbffr P&A RECRUITING LLC
