Job Description:

As a Social Worker Case Manager, you will provide comprehensive social work services in an acute care hospital setting including discharge planning, end of life conversations, and facilitating difficult placements. You will work closely with patients across various departments such as Medical Surgical, ICU, and Labor and Delivery, ensuring the effective coordination of care and support.

Responsibilities:

• Conduct drug alcohol screenings (SBIRT), mental health evaluations (SAFE-T), and capacity evaluations
• Develop behavior plans and manage BH referrals
• Assist with or provide telepsych evaluations and manage discharge planning
• Handle Single Bed Certifications/ITA processes and execute insurance authorizations

Requirements:

• Minimum of 2 years acute care hospital experience or 1 year acute care and 1 year in Hospice, Home Health, or working with the elderly
• Required Certifications: MSW, LICSW or LSWAIC (only if supervision by MHS is not needed)
• Proficiency in EPIC, with experience in home health, hospice, SNFs, Medicaid/Medicare
