Social Worker for Nursing Home

Qualifications:

• Nursing Home or Hospital experience. Intership is sufficient. (Required)

• BSW or MSW in related field. (Required)

• Bi-lingual English/Spanish (preferred but not required)

Social Worker Duties and Responsibilities:

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs

• Initiating appropriate interventions for residents experiencing mental or behavioral health issues

• Identifying individual social and emotional needs

• Participate in the planning of the resident’s admission, community re-entry, or transfer to another facility

• Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stay

• Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community
