Blended Case Manager

Position Summary - Blended Case Manager:  Enjoy helping others while working a flexible schedule, receiving great benefits, compensation for mileage, and being a part of a supportive working environment? Then this is the position for you! The Blended Case Manager provides case management services to adults and children who suffer from a serious mental illness or emotional disturbance. An assigned case manager works with the consumer and families to access, link, coordinate and monitor needed services as well as to provide support and advocacy to assist consumers in maintaining in their home or community.

The person that fills this position will need to be comfortable working flexible hours and willing to travel throughout Butler county. Services are delivered to our clients in their homes, schools/educational settings, and/or within the community. The average caseload for this position is approximately 30 clients.

EDUCATION, QUALIFICATIONS, AND EXPERIENCE:

1. Must meet one of the qualifications (a, b, c or d): a) Bachelor’s Degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling or education; OR  b) be a registered nurse; OR  c) a high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years’ experience in direct contact with mental health consumers; OR d) a high school diploma and 5 years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989^. 2. One to three years working with children and/or adults in mental health setting recommended. 3. One to three years case management experience recommended.

^Mental health direct care experience is working directly with mental health service consumers (adults, children or adolescents) providing services involving casework or case management, individual or group therapy, crisis intervention, early intervention, vocational training, residential care, or social rehabilitation in a mental health facility or in a facility or program that is publicly funded to provide services to mental health consumers or in a nursing home, a juvenile justice agency, or a children and adolescent service agency. 


ESSENTIAL DUTIES OF THIS POSITION

  1. Obtain and complete the service history information, initial assessment material, encounter forms, service documents, and the individualized service plan in a timely and accurate manner.
  2. Assist consumers with accessing appropriate community services, living in the least restrictive setting, obtaining basic necessities, and conducting activities of daily living.
  3. Provide on-call services as assigned and develop crisis plans for assigned consumers.
  4. Ensure the supervisor is informed regarding consumers, as well as accepting and utilizing routine supervision.
  5. Develop and maintain professional relationships in the community and with other office staff.
  6. Provide services at the level indicated by the consumer's individualized service plan and outlined by the pertinent regulations.
  7. Complete discharge documentation in a timely and accurate manner

KNOWLEDGE, SKILLS, AND ABILITIES

  • Knowledge of community mental health services, including publicly funded behavioral health systems, service delivery models across levels of care, referral and intake processes, and coordination with county, state, and community-based providers to support individuals and families with complex needs.
  • Knowledge of community resources and supports, including housing, income supports, food assistance, transportation, and social services, and the ability to effectively link individuals and families to appropriate services.
  • Knowledge of Pennsylvania public welfare systems, including Medical Assistance (Medicaid), Behavioral HealthChoices, county-administered human services, and the role of managed care organizations.
  • Understanding of mental health diagnoses, co-occurring disorders, and trauma-informed care principles, including familiarity with psychological and psychiatric evaluations, treatment recommendations, and service planning.
  • Knowledge of substance use disorders, recovery supports, and harm reduction principles, including how substance use impacts treatment planning and service coordination.
  • Knowledge of Pennsylvania mental health laws and regulations, including but not limited to the Mental Health Procedures Act (MHPA), child protective services requirements, and mandated reporter obligations under Pennsylvania law.
  • Skill in navigating complex systems and advocating for individuals and families within public systems while maintaining compliance with regulatory and ethical standards.
  • Ability to interpret clinical documentation, assessments, and reports and translate recommendations into practical, coordinated service plans.
  • Strong interpersonal, communication, and collaboration skills, including the ability to work effectively with individuals, families, providers, schools, counties, and multidisciplinary teams.
  • Strong documentation skills and the ability to navigate an EMR. 

REQUIREMENTS

  1. Must possess excellent verbal and written communication skills.
  2. Must have a valid driver’s license, acceptable driving record, and access to a reliable automobile.
  3. Ability to adapt hours to needs of position. May need to work evenings and/or weekends hours periodically including providing on-call emergency services on a rotating basis.
  4. Access to a telephone.
  5. Ability to remain organized despite pressured situations.
  6. Must be eligible for PA State Police clearance, PA Child Abuse History Clearance, FBI Clearance, Arrest/Conviction Report and Certification Form, and Act 168 Forms. 
  7. Must be able to identify with the agency’s Mission Statement and support the agency’s Core Values.



ANNUAL REQUIREMENTS
Training: Blended Case Managers are required to complete designated trainings related to their job performance annually.

PHYSICAL/EMOTIONAL REQUIREMENTS AND WORKING CONDITIONS:
  1. Must demonstrate strong emotional intelligence, including self-awareness, empathy, and interpersonal skills to effectively lead, support staff, and manage complex social dynamics.
  2. Must possess and demonstrate emotional stability to perform essential job functions.
  3. Mobility and physical strength to perform all tasks
  4. Must have audio/visual/verbal acuity to provide supervision for the clients and to perform essential job functions.
  5. Medical/Physical: This position is required to get a post-offer drug screening.
