RN Care Manager - PRN

Role Overview As a Care Manager, you are the hub of patient case management and care planning. You collaborate with care team members, implement care plans, and coordinate transitions to achieve the best possible patient outcomes while ensuring efficient use of resources. Responsibilities Develop individualized care plans with prioritized goals that reflect patient and caregiver preferences. Make outbound calls to assess patients’ current health status and identify gaps or barriers in treatment. Coordinate care for members and provide referrals to outside sources. Ensure members discharged from hospitals or emergency departments receive necessary services, including medication reconciliation. Triage symptom‑based calls per approved protocols for a wide range of patients. Enroll patients and manage alerts for various home monitoring programs. Provide approved general health information and guidance to callers from approved sources based on assessed needs. Document all workflows in EPIC for tracking and quality assessment. Deliver patient education to support self‑management of wellness and disease. Educate enrolled members on chronic and acute disease processes. Interact with LCMC Health care partners, leadership, and physicians to discuss clinical questions, concerns, strategies, and care plans to achieve quality and cost‑management objectives. Qualifications Current nursing license to practice in Louisiana (as defined by the Louisiana State Board of Nursing). Minimum of 2 years of professional nursing or care‑management experience.
