Why This Role Stands Out Focus on appropriate admissions and sustainable census —not just filling beds Leadership that understands the realities of hospital referrals and payer mix Strong internal support from clinical and operations teams Opportunity to build long-term, credible relationships with hospital partners Exceptional benefits package What You’ll Do as Clinical Liaison Evaluate and clinically screen hospital referrals for SNF appropriateness Build and maintain relationships with case managers, discharge planners, and hospital staff Serve as the bridge between hospital partners and the facility Coordinate admissions and ensure smooth, timely transitions of care Collaborate closely with internal teams to align on admissions strategy Support census growth through thoughtful, strategic placements What You Bring Clinical background ( RN, LPN, or Therapy ) preferred Experience in a skilled nursing facility, hospital case management, or admissions/liaison role Strong understanding of Medicare, Medicaid, and Managed Care Proven ability to navigate referral pressure while maintaining clinical integrity Strong relationship-building and communication skills Highly organized with the ability to manage multiple referrals at once If you’re in a role where you’re expected to “take everything” and are ready to be part of a more clinically driven, strategic admissions approach , we’d love to connect.
