Travel RN - Med/Surg - Hospice/Palliative Care

Requirements Active NJ RN license or compact license required Valid NJ driver's license required BLS certification required Minimum 2 years of RN experience in a home hospice environment required Associate's degree (ASN) or diploma with NJ RN license required 3+ years of RN experience preferred Experience working within a hospice agency or organization preferred Experience using an EMR required Strong critical thinking and communication skills required Knowledge of hospice regulations, pharmacology, and safe medication management Responsibilities Home care setting within a Hospice and Palliative Care program PM shift, 4pm to 8am, 3 shifts per week (16 hours per shift, 48 hours weekly) On-call coverage required for Morris & Sussex Counties Weekend rotation required every 3 weeks with comp days provided One summer and one winter holiday coverage required May float to sister hospital location as needed Uses Epic EMR for charting Performs patient assessments to determine hospice appropriateness and develops individualized plans of care Completes admission, discharge, and re-visit documentation with timely data transfer Provides age-appropriate patient and family education and evaluates teaching effectiveness Manages caseload including scheduling, care plan revisions, and interdisciplinary communication Attends IDT meetings every 2 weeks and monthly staff meetings Supervises HHAs according to hospice standards Maintains a safe environment through infection control and medication reconciliation practices Participates in performance improvement activities and may mentor new employees Local candidates accepted at a lower rate Travelers may stay on assignment up to one calendar year from initial start date before requiring a break Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs #J-18808-Ljbffr American Traveler
