We work to identify and remove barriers that can prevent people from receiving the care they need.Our integrated model brings together medical, dental, and behavioral health services, allowing our teams to collaborate across disciplines and connect patients with resources that support their overall health and wellbeing.As an RN at Alluvion Health, you'll have the opportunity to make a meaningful impact while caring for a diverse patient population in a collaborative outpatient setting.What You'll DoYour day may move quickly, but patient care always comes first.As a Registered Nurse, you will:Provide professional nursing care within your scope of licensure.Complete patient intake, including vital signs, health histories, screenings, and immunization review.Assess patients and communicate relevant clinical information to providers.Review patient medical records and prepare patients for appointments.Assist providers with examinations, procedures, treatments, testing, and other patient care needs.Administer medications, injections, immunizations, treatments, and testing according to provider orders and established protocols.Provide clear and compassionate patient education regarding health conditions, prevention, medications, treatment plans, and follow-up care.Support patients in understanding and participating in their care plans.Document patient encounters, assessments, treatments, medications, education, referrals, and follow-up accurately and timely in the electronic medical record.Coordinate specialty care, referrals, diagnostic services, and community resources.Facilitate communication between patients, providers, specialists, and other members of the care team.Support patient follow-up and continuity of care.Help coordinate patient flow so patients receive timely and efficient care.Respond appropriately to urgent or unexpected clinical situations.Participate in care team huddles, meetings, peer review, and Quality Improvement activities.Maintain patient confidentiality and comply with HIPAA, HITECH, clinical protocols, and organizational policies.Collaborate professionally with providers, coworkers, patients, families, and community partners.Complete other nursing responsibilities and special projects as assigned.You'll Thrive Here If You're an RN Who…Can assess a situation quickly while staying calm and focused.Communicates clearly with patients, providers, and coworkers.Enjoys being part of a busy and collaborative outpatient care team.Is organized and able to manage competing priorities without losing attention to detail.Provides compassionate care even when the day gets hectic.Treats every patient with dignity and respect.Is comfortable caring for people from a wide variety of backgrounds and life circumstances.Can work independently while knowing when collaboration or escalation is needed.Takes ownership of responsibilities and follows through.Values teamwork but doesn't need constant direction.Understands that excellent nursing involves both strong clinical skills and strong human connection.Education & QualificationsRequiredRegistered Nurse degree required.Current State of Montana Registered Nurse license or the ability to obtain licensure prior to the hire date.Current Basic Life Support (BLS) certification.Valid Montana driver's license or the ability to obtain one prior to the hire date.Strong clinical assessment, communication, organization, and documentation skills.Ability to work effectively with electronic medical record systems and other standard computer applications.PreferredBachelor of Science in Nursing (BSN).One or more years of experience in primary care, outpatient care, or a similar clinical setting.Benefits That Support YouAt Alluvion Health, we believe taking care of our employees is an important part of taking care of our community.
