This role is designed as a fully remote position, allowing you to contribute to client care from the comfort of your home or preferred workspace. Key Responsibilities\n

As a Licensed Clinical Social Worker at Adelante Life Solutions, Inc., your primary responsibilities will include:

• Acting as a liaison for patients, families, and outpatient providers to ensure a seamless continuum of care.

• Engaging with insurance companies to manage pre-certifications and utilization review processes, advocating for the patient's best interests.

• Contributing to discharge planning, ensuring patients have the necessary resources for aftercare.

• Providing therapeutic meal support, guiding clients in cultivating a healthy relationship with food and nutrition.

• Delivering empathetic, affirming care tailored to each client's unique circumstances and intersecting identities, promoting their dignity and well-being.

Patient Population / Client Focus\n

You will primarily work with adults, including those facing terminal illnesses and their families, ensuring they receive comprehensive social work services. Understanding the emotional, social, and financial challenges that end-of-life care presents is essential, as your role will contribute significantly to enhancing the quality of life for clients during a critical time.

Qualifications\n

The ideal candidate for this position will possess the following qualifications:

• A Master's degree in Social Work from an accredited institution.

• A valid LCSW license in the state of Nevada.

• Demonstrated experience working with older adults, with a focus on end-of-life issues.

Compensation and Benefits\n
